- Please complete any Center for Pain Management forms that were either mailed to you or given to you by the referring physician's office.
- You will receive a complete health history and physical examination.
- Laboratory tests may be required if not previously completed by another physician.
- Please remember to bring copies of X-rays, CT Scans and MRI's, if available. Please DO NOT mail these. One of the above tests may be required for review by our team of pain management physicians.
- If you are scheduled for a procedure such as a nerve block, you may be required to not consume food or liquids, including water, six (6) hours prior to your procedure.

- If you are scheduled for a procedure, you also may be required to have someone to drive you home after the procedure.
- If you are currently on any type of blood thinner medication, including aspirin or coumadin, please contact our office for instructions prior to the procedure.
- The duration of each visit varies by patient needs, you should allow two hours for the initial visit.
- In some cases, hospitalization may be indicated. The Center for Pain Management physicians have medical staff privileges at St. Marys Health Center. Additionally, they provide anesthesia services to all free-standing surgery centers in Jefferson City including JCMG and Central Missouri Medical Park.

- If a procedure is performed, there may be some discomfort during the procedure.
- You may also experience increased pain 3-5 days after an injection.
- Please bring a list of medications that you are presently taking.
- It is the patient's responsibility to make sure you contact the Center for Pain Management if you change pharmacies or if you run out of medication prior to your next appointment. Please allow three (3) business days, during Monday through Friday, to resolve these issues. Please call the Center before noon (12:00 pm) to ensure refill issues can be resolved in a timely manner.

- Patients who require narcotics for their condition will be required to sign a narcotic contract, which outlines the responsibilities of the physician and patient regarding prescription of these medications.
- For safety reasons, patients are required to come to the office for evaluation by a physician before we can authorize a narcotic refill.

- We accept all forms of insurance, including Worker's Compensation, Medicaid and Medicare. We also provide care for patients who do not have insurance and will work with you to manage your account.
- Please bring your insurance card. Insurance does not guarantee benefits, so please familiarize yourself with the pain management benefits of your particular plan.

- If you have Worker's Compensation, then please bring the insurance carrier's name, address, phone number as well as your claim number. We also need the name of your case manager and daytime phone number.
- It is the patient's responsibility to obtain a necessary referral from their primary care physician. Patients with HMO coverage are required to bring the referral to each visit. This is important and necessary to avoid cancellation and rescheduling of your appointment.

- Any and all co-pays required by your insurance plan are due at the time of service.
- Please note, all outstanding balances are the responsibility of the patient and need to be zero balances or kept current based on you special arrangements with our billing office. We will do everything we can to help you manage your account.

Each Pain Management physician pursues at least 25 hours of continuing medical education on an annual basis. The physicians maintain membership in several professional societies, which help them stay current in the highly specialized field of pain management.

These societies include: The American Academy of Pain Management, The American Pain Society, The American Pain Foundation & The American Chronic Pain Association. Members of the American Society of Anesthesiologists, Center for Pain Management physicians subscribe to its practice guidelines for chronic pain management.
